Post-partum suprapubic pain, a diagnostic dilemma in primary care setting

Osteitis pubis is a rarely reported condition in postpartum following a normal, uncomplicated vaginal delivery. A detailed history and physical examination, including the institution of appropriate management and specific diagnostic tests if needed, can aid in timely and accurate diagnosis with a good outcome. Suprapubic pain is a common presentation in the postpartum period, and many primary care doctors may not regard the pain as a serious complication. It is crucial for the primary care physician to be aware of this frustrating illness and proactively manage this condition for an effective outcome. Unresolved, prolonged pain might create unnecessary anxiety and frustrations to the mother and treating physician. The case here has been highlighted to address the diagnostic dilemma, increasing the likelihood of a precise diagnosis, particularly in the primary care setting. We present a case of a 31-year-old woman with a complaint of persistent suprapubic pain since her second childbirth for three months despite multiple visits to the local community clinic. Physical examination revealed marked tenderness over the symphysis pubis with no other significant findings. Radiological images showed evidence of osteitis pubis. She was treated conservatively but objectively with rest, NSAIDS and physical therapy. Her condition improved with time and she returned to her normal daily living activity after two months. This case emphasized the importance of recognizing and making an accurate diagnosis of postpartum pubic pain in a primary care setting and instituting the proper treatment regime early and strictly.
